Performing shipsboard jobs - enclosed space entry permit

In a ship entry into an enclosed space involve many hazards and careful consideration will need to be made for safe planning and execution. Summarized below are some basic check points. These procedures are only indicative, not exhaustive in nature and one must always be guided by practices of good seamanship

Pre-Entry Precautions (to be completed by responsible person):
  1. Have valves on all pipelines serving the space been secured to prevent their accidental opening?
  2. Has the space been cleaned?
  3. Has the space been thoroughly ventilated?
  4. Pre-entry atmosphere tests: Oxygen:
  5. % vol, Hydrocarbon: % LEL, Toxic Gas: Yes / No
  6. Have arrangements been made for frequent atmosphere checks during occupation and work breaks?
  7. Have arrangements been made for continuous ventilation during occupation and work breaks?
  8. Is adequate illumination provided?
  9. Is rescue and resuscitation equipment available for immediate use by the entrance to the space?
  10. Has a responsible person been designated to standby the entrance to the space?
  11. Has the officer of the watch been advised of the planned entry?
  12. Has a system of communication between person at the entrance and those entering been agreed & tested?
  13. Are emergency and evacuation procedures established and understood?
  14. Is there a system for keeping track of and recording who is in the space?
  15. Is all equipment used of an approved type?

Pre-Entry Checks (to be completed by team leader entering the enclosed space):
  1. All above pre-entry precautions have been taken.
  2. I am aware that the space must be evacuated immediately in case of ventilation or atmosphere test failure.
  3. I have agreed to the communication procedure.
  4. I have agreed to a reporting interval of mins.
  5. Emergency and evacuation procedures have been agreed and are understood.
